Job Description

Candidate can assess the patient's healthcare needs and work with the healthcare team to develop a care plan that addresses those needs.

2. Candidate can coordinate the patient's care across different healthcare settings, such as hospitals, clinics, and rehabilitation facilities, to ensure a smooth transition between each setting.

4. Candidate must know multidisciplinary care across variety of healthcare providers managing patients co morbidities

5. Nurse case managers monitor the patient's progress and adjust the care plan as needed to ensure optimal outcomes.

6.  Nurse should know how to take care patients aging at home, pre operative care , post operative care

7. Nurse case managers ensure that patient care is consistent with established standards of care and regulatory requirements.

8. Work with a team of medical professionals to develop and implement a comprehensive care plan based on the patient's illness and medical history.

9. Coordinate doctor's appointments and schedule surgeries (if any).
